Pending formulary changes for Altus AFB Pharmacy

Altus AFB Pharmacy has recently initiated major formulary changes based on changes to the Uniform Formulary implemented through the Department of Defense Pharmacy and Therapeutics (P&T) Committee. The P&T Committee is composed of physicians, pharmacists and representatives from the Services and the Veterans Administration.
The P&T Committee has readdressed two major classes of medications that will impact patients - acid reflux medications and cholesterol medications. Please note that the information provided will only address those medications that will be removed from the current formulary.
Aciphex, Prevacid and Tricor will no longer be formulary at Altus AFB pharmacy. Patients opting to have this medication filled at local or the mail order pharmacies will have copays of $22/month retail and $22/3-months supply through the mail order pharmacy. Altus AFB pharmacy does have an alternative for these medications in stock. The alternatives are Nexium, Omeprazole or Prilosec and Triglide.
What does this mean for patients? Patients will need to contact their providers for formulary alternatives. Be assured that no patient will go without their medication, and processes are in place to ease the transition for both patients and providers.
