Civilians eligible for unemployment claims Published Oct. 4, 2013 By Staff Notes 97th Air Mobility Wing Public Affairs ALTUS AIR FORCE BASE, Okla. -- -- Employees affected by the partial government shutdown can file an unemployment claim at the Altus Workforce Center on 1115 N. Spurgeon. Here are some tips for completing the claims process from Jennifer Bogle, 97th Force Support Squadron civilian personnel officer: - Applications can be completed online or by phone and fax. - Employees applying by phone should call 580-482-3262 and fax their documents to 405-962-7524. Include full name, social security number with a statement saying that the attached documents are "proof of earnings" on the cover page. - Employees should list their return to work date as Oct. 30, 2013. - Select "lack of work" as the reason for filing the claim. - To speed up the process, employees should provide their latest leave and earnings statement. - The SF-8 forms may be picked up in the civilian personnel office Building 52, (or outside the office if they are closed). - The state may recoup benefits should furloughed employees receive back pay.
